Welcome to the Financial Aid Request page of Hope Counseling Centers. Here you will find information on our financial assistance program for services.
Hope Counseling Centers offers a sliding scale program for clients that are in need of financial assistance. This program is available for those who do not have any other health insurance or benefits. To apply for this program, simply print and complete the Financial Aid Request Form on this page and submit at our Lakeland or Winter Haven office.
You can also e-mail the completed form to Hope Counseling Center E-Mail or fax the completed form to (863)-709-8118.
Following the submission of the Financial Aid Request form, your application will be reviewed and you will be contacted regarding your qualification for benefits.
